One proven approach is the Community Paramedic Program. These programs are tailored to meet the specific needs of each community, and Davie County EMS has developed its own program to enhance patient care and reduce healthcare costs.
Our goals are to improve health outcomes, reduce unnecessary hospital admissions, and help residents maintain safe, healthy, and independent living – all at no cost to the patient.
What types of services does the Community Paramedic Program provide?
Our program includes, but is not limited to:
- In-home visits and health assessments
- Home safety checks
- Patient education on emergency response, injury prevention, and chronic disease management
- Medication organization and reconciliation
- Vital sign monitoring and in-home lab draws (as ordered by the physician)
- Updating 911 information (File of Life)
- Pharmacy pick-up and community resource referrals
- Motivational health coaching
- Hospital discharge follow-up for patients with chronic conditions
